We are looking to recruit enthusiastic and confident nurses for the newly formed Severe Multiple Disadvantage Clinical Team.
The post is open to those with an experienced nursing background that has or would like to develop specialist knowledge working with patients that are experiencing homelessness and multi disadvantage. This post will require advanced nursing assessment skills. A background of mental health and physical health training would also be advantageous.
The post holder will be an intrinsic part of a multi agency team delivering primary care and responsible for leading the delivery to complex patients, directing their care collaboratively with multiple other agencies.

This is a new team who will be based with the Alcohol Long Term Conditions team who have bases at Park House and Stapleford. The SMD team will initially work in Mansfield and Ashfield and Bassetlaw these areas may change dependant on need.
The successful applicant will work in direct partnership with Street Outreach Team and the multi-agency case conference meeting for SMD (MEAM) to identify individuals who would most benefit from interventions provided by the SMD Clinical Team.
To deliver a nurse led case management and multidisciplinary model to ensure the health needs (acute and chronic) of people experiencing homelessness are met in order to reduce barriers to accessing health and to promote wellness, prevent ill health and early mortality.
To use advanced nursing skills and knowledge to holistically assess patients’ needs which can include unmet physical health problems ( both acute and long term ), mental health needs, chaotic drug and alcohol use and to provide clinical treatments on evidence based practice.
We have the potential for two (2) full time roles at Band 6 : Working Monday – Friday 08:00 – 18:00pm over a 4 day working week, exclusive of bank holidays. Some flexibility will be required to work outside of these hours due to the nature of the role.
Supporting the efficient running of these services are administrative and managerial roles. All roles , at all levels, are vital in transforming community health services, ensuring high quality care is provided both now and into the future based around the needs of our patients - together we all make a difference.

Applicants must be 1st Level Registered Nurse with current NMC Registration.
Role requires a registered nurse with specialist skills to assess patient needs, analyse and develop a plan of care within a multidisciplinary framework.
To provide realistic and occasionally intensive support to patients within a community setting.
The role requires excellent communication skills and the ability to develop and build relationships with partner agencies.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010
