Description:
An exciting opportunity has arisen for a dietitian with a strong interest in Community Dietetics & Public Health to work as part of the ABUHB Community Dietetics Team. We are an enthusiastic multidisciplinary team with a strong ethos of mentoring and development.
The post holder will support, develop and deliver the specialist dietetic services as part of the Community Dietetic Team. The post holder will be expected to offer dietetic clinics, group sessions and provide specialist care to a wide variety of patients within community settings. The individual gain an in-depth knowledge of the management of patients within this specialist area. The below specialist areas will form part of the successful candidates work plan;
Diabetes group education
General Dietetic Clinics
Care home nutrition support
Public health education
Supporting patients within Community hospitals
Domiciliary patient visits
The post holder will require advanced communication skills including motivational interviewing to provide a specialist service for the patients as well as being able to effectively report on the service to key stakeholders.
This is a developmental role where competencies will be developed over a 12 month period to enable the successful participant to advance into a band 6 role within the team.

Below is an outline of some main duties
To plan and prioritise own patient workload.
Assessment of individual patients’ nutritional status, requirements and intake incorporating complex data including medical history,
Assimilation of information in the context of the individual’s personal, social and clinical circumstances to provide appropriate and achievable nutritional treatment.
Implementation and monitoring of appropriate nutritional support to those patients not meeting their nutritional requirements.
Effective communication of relevant information and specialist advice to patients, parent/carers, relatives and healthcare professionals, to ensure understanding of condition and treatment.
Use of behaviour modification techniques, counselling and negotiation skills to agree appropriate goals and follow up arrangements.
To use communication, empathy and reassurance skills to foster long term good relationships with patients/carers to build trusting patient/Dietitian interactions .
Provide nutritional information as part of the formulation and review of inpatient care plans in weekly multidisciplinary team meetings.
Communicate appropriately with relevant health and social care professionals regarding planned treatment, progress and discharge of patients to ensure dietary treatment is integrated into overall care and treatment plans.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
