Senior Discharge Liaison Officer

The post holder will be working in a Multi-Disciplinary Team environment, therefore must have excellent communication skills as they will be required to liaise with teams within Medway NHS Foundation Trust and other hospital sites To assist with a multi-disciplinary and multi-professional approach to care and discharge planning, including the patient and carer in the provision of seamless care. To liaise with team, ward and health and social care professionals as necessary regarding triaging and assessment of patients and care provided, highlighting any variations to expected outcomes from treatment programmes. To be able to motivate, negotiate and persuade patients to participate in the decision making regarding their discharge destination as part of the patient discharge pathway. To assess patients progress and within the parameters set by the therapist and or nurses and use of both analytical and judgemental skills to make suggestions and or alterations to the existing discharge plan to enable the patients journey to progress whilst liaising with all relevant professionals To undertake triage and assessment of patients with a wide variety of presentations and conditions: using clinical reasoning skills and assessment techniques without supervision, discussing your findings with the relevant cluster team staff member
