Description:

A fantastic opportunity has arisen for a Band 7 Senior Liaison Practitioner to join the new High Intensity User ( HIU ) service in Newham, based in the Psychiatric Liaison Team at Newham University Hospital. The service operates from 9-5pm, Monday to Friday, and is a service designed to support individuals who frequently attend A&E.
The HIU service aims to review a cohort of people who are frequently attending the A&E at Newham Hospital and provide clear guidance and specific personalised care plans for some of the most complex patients who attend A&E.
The post holder will be expected to be self-motivating and autonomous with the ability to demonstrate accurate formulations of mental state, risk and need for people over the age of 18 years taking into account medical, psychological and social factors and referring on to other disciplines or other agencies as appropriate.
The post holder will provide advice on mental health issues and participate in development of the care plan for the person who is frequent attending.
Provide a forum for multi-disciplinary meetings to discuss complex patients who
presents frequently to A&E
Create personalized and first-person care plans for specific frequent attendees.
Where this is not possible care plans bearing the person and their complexities in mind are created
Provide expert advice on the management, treatment and diagnosis of patients to acute hospital clinicians – where appropriate for HIU to seek expert advice
from the relevant clinical speciality such as haematology, renal etc.
Provide support to acute hospital clinicians to understand the complexities of frequent attending.
To reduce readmissions to the Emergency Department
To improve the experience of patients who receive care in the acute hospital.
