Description:

An exciting opportunity has arisen for a permanent Band 6 Senior Palliative Care Nurse 18.75pw to join the Palliative Care Service in Northumbria Healthcare Trust.
The post will be part of the Palliative Care Team across both South East and Central Northumberland. The successful candidate will be a vital member of a multidisciplinary team. The Team includes Consultants, Nurses, Occupational Therapists, Physiotherapist, Technical Instructors and Healthcare Assistants.
The post will be based at Nursery Park Primary Care Centre in Ashington although in times of need may be required to cover other areas across Northumberland and North Tyneside.
The post holder will demonstrate management of patients with complex life limiting illnesses within a community setting and will work in partnership to enable patients to be cared for in their preferred place. The post holder must be able to demonstrate that they have the ability to work as an integral member of the multidisciplinary team and have excellent leadership and communication skills. Palliative care experience would be essential. The post holder must have a valid driver’s licence and access to a vehicle to be used for work purposes.
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate. To provide specialist palliative care for patient and carers with life limiting illnesses within the community settings.
To develop partnerships with members of Multidisciplinary Team in the Trust in order to provide a seamless service of palliative care, to enable the patient to be maintained in an environment of their choice.
To be a resource in all settings for palliative care services.
To provide clinical professional support, resources and education when necessary to members of the Multidisciplinary Team.
To work as within a multidisciplinary environment, ensuring adherence to professional codes of conduct.
To work with other professionals to promote and develop the ethos of Palliative Care within the Trust and the local community.
To participate in appropriate research programmes and clinical audit activity as a self directed professional in collaboration with other members of the interdisciplinary team contributing to advancement of nursing and medical knowledge.
To practice as a self-directed practitioner, autonomously organising and accountable for a clinical caseload reflecting core components of the clinical nurse specialist role.
To maintain accurate patient records, adhering to strict patient and family confidentiality, as required by N.M.C and the Trust and to maintain accurate data, and compile statistical information profiling the service and informing strategic development.
To lead on specific projects as agreed within the team and as specified by National Service Frameworks
