About the role

Permanent and fixed term temporary full time or part time opportunities available.

• Deliver high level clinical pharmacy services and leadership across the breadth of STARS services and alongside the multidisciplinary team.
• Contribute to practice and service development, as well as innovative activities that improve medicines management for patients accessing STARS services, that optimise electronic medicines management systems, and that are aligned to organisational goals, professional, legislative and ethical standards.
• Exercise independent professional judgement in resolving problems with more complex clinical decisions and problem solving made with limited professional supervision and guidance.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health’s values:

• Demonstrate a high level of knowledge, expertise and skill in Pharmacy practice including the use of digital platforms for innovative hospital pharmacy service delivery
• Provide accurate, timely and appropriate clinical information and implement liaison systems with hospital staff, General Practitioners, community pharmacies and residential care facilities, to optimise continuity of care.
• Contribute to the development and maintenance of clinical practices, procedures and protocols that support the continuum of care incorporating an interdisciplinary focus as well as the standardisation and optimisation of digital systems.
