Description:
Job Purpose
To ensure delivery of an effective and efficient physiotherapy service across the acute floor: Acute Medical Assessment Unit, Acute Medical Short Stay Unit and Emergency Department (ED)/Frailty Intervention Team (FIT)
To work closely with the multidisciplinary teams across each area (MDT) in the provision of patient-centred care for patients across the acute floor.
To ensure a seamless transition of care for patients admitted from ED to both the acute and to the inpatient wards.
To work as a member of both the Acute Medicine MDT, FIT Team and Emergency Medicine team to provide standardised high quality physiotherapy care to patients referred to each service.
To provide physiotherapy leadership across the acute floor.
To work across existing acute floor services to reduce patient experience times, to reduce inappropriate hospital admissions and to reduce hospital length of stay, in line with the key objectives of the National Acute Medicine Programme, National Emergency Medicine Programme and the National Clinical Programme for Older Persons (NCPOP).
To complement existing inpatient to provide timely, safe and high quality clinical care to all referred patients.
To work with colleagues to undertake and facilitate relevant research in the clinical area, in line with hospital policies and to present and publish research outcomes.
To work in an advisory capacity and as a central resource to therapy, medical directorates, regional or national agencies, as required.
To complete service development, using evidence based practice, audit, lean methodology, risk assessment and implementation of change as required.
To deliver education of the physiotherapy team and support education and training staff as well as facilitating clinical education of undergraduate and post-graduate students
To undertake and facilitate relevant audit and research in the clinical area, in line with hospital policies and to present and publish research outcomes.
To work closely with community colleagues, rehabilitation sites, Mater Minor Injury Unit (Smithfield) and other stakeholders involved in patients care to ensure use of alternative care pathways to avoid unnecessary hospital admissions.
