Description:

This is an exciting opportunity for an ambitious senior manager to be at the heart of the development and delivery of initiatives for Primary and Community Care, in line with SWL ICB and Croydon Place based priorities. The overarching focus of the role is to improve access, experience and health outcomes through
integrated care delivery across providers in the community.
The successful candidate will have experience in transforming and integrating health and care services, achieving change through partnership working. They will have a good understanding of NHS and care services, commissioning, programme management, community development and the primary care strategic development agenda.

  • The post holder will lead on transformation across primary care, community health services and development of integrated care delivery across all local health and care providers for out of hospital services, ensuring alignment with the local Health and Care plan and the One Croydon Alliance priorities.
  • Lead the delivery of the wider prevention, proactive care and access improvement objectives as set out in the SWL primary care Strategy.
  • Ensure good local and regional governance for decision making and for the development and implementation of delivery plans.

The post holder will be a member of the Croydon Primary and Community Development and Delivery Team and contribute to the Place team’s objectives and deliverables; working closely with the local NHS Trusts, Council, primary care, community health providers and the VCSE sector; as well as working closely with colleagues across Southwest London ICS.
NHS South West London Integrated Care Board (ICB), as part of South West London Integrated Care System (ICS), is a partnership of organisations that come together to plan and deliver joined up health and care services to improve the lives of people in our six boroughs: Croydon, Merton, Kingston, Richmond, Sutton and Wandsworth.

EACH ICS CONSISTS OF TWO STATUTORY ELEMENTS:

an Integrated Care Board, bringing the NHS together with its partners locally to improve health and care services
an Integrated Care Partnership (ICP): the broad alliance of organisations and representatives concerned with improving the care, health and wellbeing of the population, jointly convened by the ICB and local authorities in the area.
ICBs are statutory NHS bodies responsible for planning and allocating resources to meet the four core purposes of integrated care systems (ICSs):
to improve outcomes in population health and healthcare;
tackle inequalities in outcomes, experience and access;
enhance productivity and value for money and;
help the NHS support broader social and economic development.
NHS South West London Integrated Care Board decides how the South West London NHS budget is spent and develops plans to improve people’s health, deliver higher quality care, and better value for money.

Duties and Responsibilities

  • Lead a local team to develop high quality integrated primary and community based

services, ensuring equitable services are available for the whole population, whilst

adapting for local need, using health insights from population health tools.

  • Work closely with the Croydon Place team to implement national, South West London

and Croydon local strategies and priorities, including the Croydon Health and Care
Plan, the Croydon Clinical Strategy and Local Objectives. This will require working with
colleagues across the health and care system to translate national and ICS
programmes into local schemes, developing business cases and project plans,
allocating tasks as appropriate, identifying risks, issues and dependencies,

considering best practice and options and monitoring the progress of programmes.

  • Lead on the management and delivery of key projects within the Croydon Primary and

Community Care Development and Delivery portfolio, ensuring that results are

delivered within deadlines and to a high standard.

  • Provide strong project management, producing clear plans, setting direction and

ensuring successful delivery on time, to high quality standards and in a cost effective

manner.

  • Plan, develop and evaluate methods and processes for gathering, analysing,

interpreting and presenting accurate and timely data and information
Ensure flexibility of the programme to meet conflicting/changing requirements if

required.

  • Demonstrate effective stakeholder management across the system and at all levels,

with in-depth knowledge to be able to respond to and resolve conflict between different

providers when this arises, through facilitation or other appropriate mechanisms.

  • Consider the impact of any change on existing contract arrangements and oversee

any necessary changes, ensuring full ownership and buy-in of providers at senior and

executive levels.

  • Work closely with practices and community services to develop and maintain a firm

understanding of performance and sustainability issues, including practice
sustainability and vulnerability, providing advice, support and guidance to mitigate

risks, as appropriate.

  • Work closely with One Croydon Alliance partners, including Croydon Council, Croydon

Health Services, South London and Maudsley NHS, the Croydon GP Collaborative

and Croydon based Voluntary Sector, social enterprise and faith organisations.

  • The role will require supporting local governance arrangements, including working with

system partner to manage local governance meeting relevant to this post.

  • The role is designed to build a combination of subject matter expertise and technical

skills to develop strong service delivery
