Description:
Contribute to the teams engagement with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies. Impart complex and sensitive information to patients and families/carers re assessment, care planning, treatment and review. Multi-disciplinary teams to ensure the provision of consistent well-coordinated care. External agencies also involved in the provision of care eg: voluntary and private sector, police, social services, housing, employment etc.
Service user/carer groups. Liaise with a wide range of other professionals and agencies. Uses a high level of communication and interpersonal skills to aid overcoming barriers to acceptance, such as would be required when communicating in a hostile antagonistic or highly emotive atmosphere. Present information some of which may be contentious to patients and their families/carers.
To take part in formulation and clinical review of care and treatment plans. Making judgments about complex facts or situations requiring analysis, interpretation, comparisons of a range of options.To work with the clinical team lead in making a judgement in patient management matters. Responsible for developing care packages including assessing, planning, intervention and evaluation. Ensuring active involvement of patients, families and carers.
Undertaking assessments regarding current social circumstances and the impact of social issues on patients mental health and recovery Liaising with a host of external agencies to enable them to fulfil their statutory duties, for example: Safeguarding, MAPPA, Local Authority Social Services, Police and Probation. Supporting patients in developing feasible plans for the future and working towards discharge. Supporting carers and families of detained patients. Taking a lead role within the multi-disciplinary teams on non-medical mental capacity issues and best interest decisions.
Ensure effective care pathways are in place for transition between mental health and other services. Promote, develop and role model high standards of clinical care. To ensure that robust, up to date risk assessments are carried out by the relevant staff. To ensure standards of care are maintained to the highest level.
To promote good practice in the management of patient risk management and safety. Promote a duty of care to users of services; encompassing safe and competent care Peoples wishes, feelings, values and beliefs should be taken into consideration alongside the views of those close to them. Take personal accountability for own practice; answerable for actions and omissions, regardless of advice or directions from another professional
