We offer a comprehensive gamut of services to care for the sick, frail and elderly who have physical or mental disabilities with minimal or no appropriate caregiver support at home.
The Home has 14 separate Resident Living Areas (RLAs), each designed as an individual home-like environment. Hope RLA is specially designed to meet the needs of relatively independent residents with dementia.

  • Bachelor Degree/Graduate Diploma/Masters in Social Work is a mandatory

Responsibilities:

KEY RESPONSIBILITIES

  • Support the Active Ageing Centre’s (AAC) provision of active ageing, befriending & buddying, care referral services to seniors within centre’s service boundary area.
  • Work closely with centre’s Programme team in identifying seniors who may require additional care referral services and assist in raising appropriate referrals to care and support services.
  • Receive and review referrals for CREST service in conjunction with CREST team/Centre Manager to determine needs of clients and/or caregiver(s). Inform and consult CREST team/Centre Manager of recommendations regarding admission and classification of case (P1-3).
  • Provide case management services to clients and/or caregiver(s) under CREST programme through providing basic emotional support, monitoring, service linkages, to assist them to manage their mental health issues and caregiver stressors.
  • Work closely with primary care (eg. GPs, Polyclinics), health care providers (eg. RHs), social service agencies (eg. FSCs, SSO) and other community partners (AACs, SGO, PA, CC/RC) to identify at risk cases in the community within Fengshan division. To conduct joint assessments to assess needs and suitability for enrolment into CREST programme.
  • Receive and review referrals for Dementia Day Care programme in conjunction with Nursing Department to determine suitability and appropriate placement in the centre. Inform and seek approval from Centre Manager of recommendations regarding admission and placement.
  • Present admissions application for Dementia Day Care programme to Dept Head/ED for final approval of funding and placement of applicant.
  • Provide social work related support services to clients and their families, e.g. referrals to community or residential services.
  • Upon referral from both government/non-government sources implement an appropriate system for assessing and investigating current and future applications
  • On admission work closely with client and family to orient them to the centre and facilities.
  • Develop an appropriate internal and external network to assist in resourcing and referral of service users.
  • Work closely with the multidisciplinary team to ensure that psychosocial needs of the clients and their caregivers are actively considered in their care management.
  • Ensure “hands-on” approach to working with service users.
  • Maintain accurate and current documentation of casework for assigned caseload.
  • In compliance with government regulations for continued funding, conduct annual review of household incomes/social conditions of residents’ support network.
  • Work closely with peers within department to maintain an efficient and effective Social Work department.
  • Participate in multidisciplinary meetings and work closely with Case Manager (Clinical) in discharge planning reviews.
  • Attend case conferences, meetings, network with external agencies as required.
  • Be a resource person for specific projects as needed.
  • Undertake any other related duties considered necessary for effective programme support.
