JOB REQUIREMENTS
- Registered Nurse with Singapore Nursing Board
- Minimum 3 years’ experience working in acute hospital settings and or familiar with Community hospital settings
- Candidates with Advance Diploma in nursing specialties preferred
- Able to be work on extended hours (5pm to 9pm) on certain weekdays, on rotational basis,estimates 3 to 4 times a month
- Must be comfortable to travel around on a daily basis
- Head office: Toa payoh
- Monday to Friday

Responsibilities:
- Provide coordination and care management for assigned patients
- Connect patient with community resource and financial assistance
- Perform nursing procedures in home environment
- Develop and evaluate care plan and goals
- Liaise with other healthcare partners who are caring for the same patient
- Participate in multidisciplinary round
- Maintain timely and accurate documentation of patients’ condition and care given
- Provide care giver training programme and health education to patients / caregivers and evaluate the teaching programmes to meet patients and caregivers needs and expectations.
- Participate in quality improvement project and nursing research
