Come and join our Integrated Care Team –Therapy.
We have a full-time Senior Therapy Assistant vacancy to work in a community therapy team in the South of the City. The Team treats adults over 16 referred from the Hospital, GPs, District Nurses, and other Health and Social Care services with a variety of conditions and therapy needs to maintaining and improve independent living.
The successful candidate will deliver rehabilitation programmes and provide equipment to patients within the home environment. You will work alongside occupational therapist, physiotherapists and specialist therapy assistants to progress rehabilitation and improve independence.
We work in an integrated way with the nursing teams to achieve the patient goals in a patient centred way. Staff are well supported with regular team meeting, supervision and training.
To assist the interdisciplinary team in the provision of patient centred care.
To implement packages of care and rehabilitation programmes in the community and complete patient records.
Undertake assessment of patients within departmental guidelines.
To assist the interdisciplinary team in maintaining an efficient working environment.
