Responsibilities:
MAIN DUTIES OF THE JOB
Working in a busy and dynamic Adult Community Mental Health Team means every day will be different. An outline of the role is as follows:
- Overseeing daily triage meetings across both services and completing triage contacts for complex cases.
- Being able to work flexibly according to the needs of the service.
- Providing clinical leadership and clinical supervision to practitioners completing triage and assessment contacts.
- Undertaking initial urgent/routine assessments on behalf of the service
- Chairing and leading multi-disciplinary team meetings with our Consultant Psychiatrists, Nurse Consultants, Clinical Team Managers, Senior Practitioners, Assistant Practitioners and Allied Health Professionals including Consultant Clinical Psychologists, Clinical Associate Psychologists and Assistant Psychologists
- Attending outpatient clinic appointments
- Undertaking face to face visits in the community setting including home visits
- Working in collaboration with internal/external stakeholders and attending multi-agency meetings where appropriate.
DETAILED JOB DESCRIPTION AND MAIN RESPONSIBILITIES
You will need to be enthusiastic and conscientious and do everything you can to provide the best care to our service users. We are looking for someone who displays compassion, empathy, and sensitivity in your approach to people who may be distressed or be presenting with a complex or challenging condition. You will be passionate about ensuring service users receive excellent quality assessments whilst under the CMHT.
The role will require confidence in providing clinical leadership and direction to practitioners within the team working with service users at the triage and assessment stage of their journey with mental health services. For service users presenting with complex conditions, you will complete initial assessments and triage contacts to determine the appropriate needs for the individual.
You will be able to work with external stakeholders as well as members of the multidisciplinary team, to ensure service users do not experience delays in accessing their treatment.
