Description:

SECTION 1- MAIN DUTIES & RESPONSIBILITIES: The post holder must demonstrate competence in a wide range of expanded roles including: Venepuncture, cannulation, ECG recording & basic interpretation, plastering, requesting and obtaining pathology samples, utilising PGDs, requesting xrays. The post holder must be able to demonstrate a high level of clinical decision making skills. Excellent communication skills are required to ensure that patients, relatives and the multi-disciplinary team are able to understand the often complex needs of the patients. To complete Trust documentation/liaise with all agencies if any suspicions arise regarding the possibility of any Safeguarding Adult/Children issues.
Maintain effective communication within the team by ensuring that information is transmitted at handover and is documented either in the notes, appropriate folders or on the computer system as deemed necessary. All documentation relating to a patients episode within the department is to be recorded accurately in the patients notes/computer system for future referencing as required. Assist in maintaining a calm, efficient and cost effective service. Assist in Co-ordinate/facilitate new staffs orientation into the department.
Undertake individual responsibilities as delegated to the post holder. PROFESSIONAL RESPONSIBILITIES: Ensure the delivery of evidence-based nursing care by providing evidenced based clinical assessment, care planning, care delivery, care evaluation and clinical advice in accordance with NMC Code of Professional Conduct To keep up-to-date with NHD developments, in particular NICE guidelines and NSFs ensuring that nursing practice within the department develops accordingly. To participate in the clinical governance agenda, audits of areas within the Urgent Care To maintain clinical practice standards and knowledge, and act as a professional role model for all nursing staff. Use own clinical judgement to initiate appropriate nursing interventions prior to medical Maintain accurate, current and contemporaneous records and ensure that all records relating to patient care are clear, concise and legible in accordance with the NMC Standards for Records and Record Keeping.
Ensure adherence to the Trust Medicines Policy and the NMC Standards for the Administration of Medicines. Assist in the leadership of specific areas of practice developments in accordance with local and national policies and strategies to improve patient care. Actively assess and manage risk within the clinical area in accordance with Health and Safety legislation and risk management policy. Work within Data Protection and Caldecott requirements for information sharing.
To maintain continuous professional registration with the NMC throughout the duration of employment with the Trust TEACHING & DEVELOPMENT: Act as a clinical supervisor to junior clinical staff completing appraisals and agreeing personal development plans ensuring systems are in place for staff to be able to reach their objectives. Promote clinical innovations and encourage evidence-based practices within the unit. Assist in the developments of components for a teaching program for use within the ADMINISTRATIVE/MANAGERIAL: Assist with duty rotas as required by the Senior Sisters/Charge Nurse. Be responsible for the authorisation of timesheets/overtime as and when required Work within clinical governance systems and ensure that care is delivered within an appropriate model within the Acute Medical Unit.
Team Leader Nurse as required. Implement and ensure adherence to all Trust policies. Develop and implement protocols and or policies within own specialist area. Effectively contribute to the achievement of Trust targets.
Support the Ward Manager and Matron for Urgent Care Participate in the identification of cost improvement initiatives with Ward Managers and Matron. HUMAN RESOURCES MANAGEMENT: Support effective intra and inter departmental communication. Provide competent and consistent leadership to the ward/department team. Undertake PDRs for junior colleagues and ensure PDPs are in place.
Participate in training and development for all staff, ensuring that accurate records are maintained. Be actively involved in the induction and mentorship for newly deployed and newly qualified staff. Ensure that educational objectives of student nurses and other learners are met and the progress of each student/learner is monitored. Participate in the management of sickness/absence.
Participate in clinical supervision in accordance with Trust policy. SECTION 2 - KNOWLEDGE AND SKILLS Possess evidenced based clinical knowledge appropriate to the Acute Medical Unit / Same Day Emergency Care. Utilise own clinical expertise and knowledge to create and promote an environment conducive to meeting the educational and developmental needs of all clinical staff. Possess courses appropriate to the Acute Medical Unit / Same Day Emergency Care.
SECTION 3 - EFFORT AND ENVIRONMENT Partake frequently in tasks requiring physical effort for long periods Regularly deal with bodily fluids and soiled linen within a span of duty. Manage and participate in distressing unpredictable situations frequently within a span of duty Maintain contemporaneous patient records at all times Concentrate for long periods and respond to unpredictable patient and staff demands. Provide and receive complex information that will require empathy, tact, reassurance and communicate in situations where there maybe barriers to understanding. Subject to verbal assault and aggression from relatives/carers within a span of duty.
Defusing of aggressive situations using complex communication skills, experience and information. Work with patients with mental health needs and liaise with relevant speciality in relation to clinical management
