We are looking to appoint a Social Care Coordinator to join our Independent Living Technology Enabled Care (TEC) Project. The TEC Project is a project (funded through the Adult Social Care Technology Fund that is exploring whether the use of Internet of Things (IoT), artificial intelligence (AI) and other disruptive technologies can support elderly and vulnerable people in Reading to live more independently.

Responsibilities:
You will work with new innovative technology to support the users to live independently, you will have a broad caseload, focused on the users using the project sensor-based technology.
Last summer Reading Council applied to the Department of Health for Digitising Social Care Funding and became one of only four organisations in the UK to receive a share of the technology funding. We are now recruiting to expand the project and looking for a Social Care Co-ordinator to work alongside our Project Manager, a Senior Occupational Therapist (OT), another Social Care Co-ordinator and a Business Systems Analyst.
We are looking for someone with experience of working with TEC and of undertaking social care assessments in the community. You will bring with you knowledge and experience of Adult Social Care as well as a customer-focused drive to ensuring the residents of Reading have the right services at the right time, catering to their needs.
The project is being run as part of the wider research project, the results of which will be fed up and shared at a national level to support ASC across the country, meaning the work you do will be felt across the country.
Social care coordinators, work with individuals to undertake assessments, reviews and care and support planning in line with Care Act 2014, Mental Capacity Act 2005, and where appropriate Mental Health Act 1983 (as amended 2007) duties.
The role entails taking a strength-based approach supporting people to use preventative technology, and to build resilience and achieve outcomes that are important to them, involving the person and supporting them to have choice and control. This will include visits to residences, explaining the system and installing it, training will be provided on this.
We are looking for candidates who have experience of working with people with care and support needs or have transferable skills and experience.
