This is a Public Services Network (PSN) role and requires all successful applicants to undergo a Basic Disclosure check and a 3 year’s verification of employment history. Please note that any start date will be dependent on the receipt of a satisfactory Disclosure and verification of employment history.

YOUR ROLE

We are looking for an experienced Social Care Practitioner to join our team. As a Social Care Practitioner in the team, you will work with residents of North East Somerset who may be in need of social care support.
You will primarily be undertaking assessments under the Care Act 2014. This involves direct work with individuals, families and professionals, report writing, support planning, Care Assessments and Mental Capacity Assessments.
