WHAT WE NEED FROM YOU
It is essential that you have experience in a relevant area of statuary or voluntary sector work in health and social care.
- You must have a relevant qualification e.g. NVQ Level 3 in health or social care field
- We require you to have excellent interpersonal skills.
- You will be able to gather information from a variety of sources and follow assessment process.
Ideally, you can demonstrate knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.
Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post
You will be expected to travel throughout the authority and surrounding area so must have a full valid driving licence with regular access to a vehicle.or have alternative appropriate means of travel.
Responsibilities:
- As a key contact for cases, you will be required to visit service users and their families/carers and undertake an assessment of need.
- You will be assessing for the provision of equipment to support individuals and help prevent, reduce and or delay care and maximise independence under the Care Act 2014
- You will be assessing the need and outlining the specification for adaptations to homes within the eligibility of the Housing Grants, Construction and Regeneration Act 1996.
- In the role, you’ll refer issues if needed in accordance with Safeguarding procedures to protect vulnerable people. This will require you to maintain confidential reports to profession and Council guidelines.
- You’ll be communicating, providing advice and guidance to service users and providers
- Daily you’ll be liaising and working with Occupational Therapist, Social Workers, Senior practitioners and external contacts.
