In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Reporting to the Manager or designate, the Social Support Worker participates in the provision of client-focused treatment, rehabilitation and support for persons with serious and persistent mental illness in a residential, community-based facility. Participates as a member of the multidisciplinary team and as directed, assumes a lead role for overseeing and monitoring designated residents and facilitating the rehabilitation of residents to the community. Monitors and documents functioning levels of the resident. Promotes independence for resident through active involvement in day-to-day activities. Participates in the Discharge Planning Process. Compiles information.
EDUCATION, TRAINING AND EXPERIENCE
College Diploma in Social Services field plus 2 years related experience working with persons with mental disorders, including teaching life skills or an equivalent combination of post secondary education training and experience. Knowledge of and experience in applying the principles of mental health treatment and psychosocial rehabilitation, using a client-centered approach. Knowledge of the Mental Health Act as it applies to the rights and obligations of clients and staff in the facility. Knowledge of government agencies, community services and resources and how to access these resources. Valid Class 5 Vehicle License

- Ability communicate effectively verbally and in writing.
- Ability to utilize government agencies and community resources effectively and to develop positive working relationships between these resources, the resident, families and other service providers.
- Ability to deal effectively with individuals with serious and persistent mental disorders.
- Ability to employ aggressive behaviour management techniques.
- Ability to apply crisis intervention and prevention skills.
- Ability to operate related equipment.
- Effective assessment, planning and organizational skills
