Description:
Principle Duties & Responsibilities:
- Plan, organise and conduct outreach and screening in the community to promote mental wellness, health awareness and resource linkage.
- Implement assessments, care planning, intervention strategies, care co-ordination, service linkage and monitoring for potential and existing clients.
- Conduct talks, workshops and training for clients, general public, volunteers and colleagues.
- Develop and maintain partnerships for community outreach and service linkages.
- Develop and maintain partnerships for community outreach and service linkages.
- Perform administrative work including admission/ discharge process of clients, maintaining up-to-date records of clients, care plans and visit logs for accurate reporting purpose.
- Support in the update of CREST service manual to align with service requirements and processes.
- Manage queries, provide information and make referrals received via phone, walk-in, Care Line or other platforms to appropriate services provided by TOUCH or other service providers.
