Advert for a Care Coordinator Social Worker Band 6, Full-time,
An excellent opportunity has become available to be part of our Community Mental Health Team. This is an ideal opportunity for a qualified professional to expand in their skills, expertise, multi-agency liaisons and autonomous practice.
We are looking for skilled and motivated mental health professionals, of a social work background, to join us in meeting the needs of those we serve in the vibrant and diverse locality of Luton.
We are a community mental health service looking for resilient and proactive team players.
We seek staff committed to a user focused approach, who is able to work creatively and effectively, in this busy and demanding role, to promote patient recovery.
The successful applicant will hold a caseload of individuals who experience severe and enduring mental ill health; many of whom will have complex needs. The focus of the role is to work responsively with an individuals strengths, in order to maximise life choices and independence.
As part of the team staff members are also required to participate in Duty, Assessment and Clinics as appropriate to your role. Working days are Monday to Friday, 9am 5pm.

  • To promote a service user focussed service.
  • Participate in audits and survey relevant to the team.
  • To participate in development of new policies and procedures as required.
  • Ensure that clients receive a culturally competent service.
  • To co-ordinate the day-to-day activity and manage the workload of the Team.
  • To ensure the legal requirements of the Mental Health Act (1983) and subsequent amendments

are adhered to.

  • To ensure the legal requirements of the Care Act 2014 other legislation
  • To establish and maintain good relationships with service users, carers and voluntary sector
