Position Summary

Excellent opportunity for an experienced Grade 2 Social Worker to be part of a dynamic and inspiring social work and broader allied health team working in a collaborative model of care across Monash Health

• Join an active, energetic and friendly Allied Health team.
• Primary based at Victorian Heart Hospital, and at times MMC & Moorabbin Hospitals
• As a heart hospital team member support inpatient services & Cardiac Rehabilitation Program
• Work in a setting assisting adult patient cohorts across Cardiology, Cardiac Surgery, Intensive Care, Cardiac Emergency & Ambulatory Cardiac Prevention & Rehabilitation Program, and other specialty areas as required
• A focus on psychosocial assessments, risk assessments, complex discharge planning, bereavement, adjustment to illness and family support.
• Availability one Tuesday per month (1pm – 9pm) to deliver education in the Cardiac Rehabilitation Program.

ABOUT THE ROLE
