Location: Boston, Lincolnshire
Team/Specialism: Hosital Discharge
Pay Rate: £21.12 per hour (assignment rate)
Contract: Locum, 3-Months
Hours: Full-time

REQUIREMENTS OF THE SOCIAL WORKER:

• Degree or equivalent in social work
• Current Social Work England registration
• Significant frontline experience in Adults Services within the UK

• To complete Adult Care assessments and facilitating timely discharges. Being mindful of the Care Act, Mental Capacity Act, Safeguarding, Continuing Health care, Human Rights etc.
• To work closely with Health partners completing board rounds and MDT meetings.
• To support patients, carer and families to navigate the discharge process.
