DETAILS

  • Schedule: Full-time, Day shift, 8:00 a.m - 4:30 p.m. with rotating weekends and holidays
  • Hospital: Ascension St. Joseph Hospital

REQUIREMENTS

Licensure / Certification / Registration:

  • Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date required.
  • Approved Local Exception preferred.
  • Drivers License preferred.
  • Licensed Clinical Social Worker (LCSW) credentialed from the Illinois Department of Financial and rofessional Regulation preferred.
  • Certified Case Manager credentialed from the American Case Management Association (ACMA) preferred.
  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

  • Master’s degree in Social Work required.

Work Experience:

  • 3 years of experience preferred.
  • Previous experience in an acute setting preferred.

Responsibilities:

Apply specialized clinical knowledge and advanced clinical skills in the areas of assessment,

diagnosis, and treatment of mental, emotional, behavioral and developmental disorders, conditions and addictions.

  • Administer and interpret tests and measure of psychosocial functioning for the purpose of diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities.
  • Develop and implement appropriate assessment-based treatment plans.
  • Consult with team managers, statutory and voluntary agencies and patient’s relatives to ensure compliance with the patient’s treatment plan.
  • Provide crisis-oriented psychotherapy as well as short-term and long-term psychotherapy and psychotherapeutic treatment.
  • Document patient therapy in accordance with both federal and state regulations.
