Social Worker MSW, Case Management

JOB OVERVIEW:

As a Social Worker, you will provide frontline services to patients with conditions spanning the entire health care continuum.
The goal for this acute care position is to discharge patients home or to another safe environment.
As an essential member of the interdisciplinary team, you will work in concert with doctors, nurses, and allied health professionals to sensitize other health care providers to the social and emotional aspects of a patient’s illness.
Utilizing your case management skills, you will help patients and their families address and resolve the social, financial and psychological problems related to their health condition.

QUALIFICATIONS:

Required:

• Master’s degree in Social Work (M.S.W.) from an accredited School of Social Work
• New Jersey State Social Work Licensure (L.S.W. or L.C.S.W.)
• Good communication and writing skills
• Ability to work in a multidisciplinary team

Preferred:

• Bilingual Spanish preferred
• Hospital experience strongly preferred

SCHEDULING REQUIREMENTS:

• Shift working, 8:30-4:30pm each day
• Monday-Friday and rotate Saturdays
• FT
