As a Senior Social Worker in the Post Adoption Team, you will play a vital role in supporting children who have been adopted and their families, ensuring that they receive the necessary support and services to thrive. You will work collaboratively with adoptive families, birth families, and other professionals to facilitate successful adoptions and provide ongoing support to families post-adoption. This role requires a compassionate and skilled social worker who is committed to promoting the well-being of children and families and empowering them to build positive futures.

Key Responsibilities:

  • Assessment and Support:
  • Conduct comprehensive assessments of the needs of adopted children and their families, taking into account their individual circumstances and experiences.
  • Provide direct support and intervention to adoptive families, offering guidance, advocacy, and access to relevant services and resources as needed.
  • Work collaboratively with birth families, adoptive families, and other professionals to facilitate contact arrangements and support positive relationships where appropriate.
  • Case Management:
  • Manage a caseload of post-adoption cases, ensuring that all statutory obligations and deadlines are met and that the needs of children and families are addressed effectively.
  • Develop and implement care plans in collaboration with adoptive families, setting clear goals and objectives and identifying appropriate support services and interventions.
  • Maintain accurate and up-to-date records of all interactions, assessments, and interventions, ensuring compliance with legal and regulatory requirements.
  • Support and Supervision:
  • Provide support and guidance to less experienced social workers and support staff within the team, offering supervision, mentoring, and professional development opportunities as needed.
  • Participate in team meetings, case conferences, and training sessions to share knowledge and expertise and contribute to the continuous improvement of service delivery.
  • Partnership Working:
  • Develop and maintain effective partnerships with relevant agencies and professionals, including local authorities, adoption agencies, health services, and educational institutions, to ensure coordinated and holistic support for adopted children and their families.
  • Advocate for the needs and interests of adopted children and their families within multi-agency settings, ensuring that their voices are heard and their rights are upheld.

Requirements:

  • Degree in Social Work or equivalent qualification recognised by Social Care Wales.
  • Registered with Social Care Wales as a Social Worker.
  • Significant post-qualification experience in a social work role, with specific experience in adoption and post-adoption support preferred.
  • Strong assessment and communication skills, with the ability to engage effectively with children, families, and professionals from diverse backgrounds.
  • Knowledge of relevant legislation, policies, and procedures related to adoption and post-adoption support.
