    Job Description:
    Facilitating continuity of care, the Social Worker, provides assessment, counselling and treatment services to patients in an acute care and inpatient rehabilitation setting. The Social Worker assists patients and families address the impact of illness and treatment; tremendous stress often stems from hospitalizations that are sudden and at times related to illness or injury.
    Social workers provide assessments and appropriate interventions to aid the patient in achieving optimum recovery and quality of life. The social workers at Cambridge Memorial Hospital have expertise in the areas of general medicine, surgery, emergency, ICU, geriatrics, obstetrics and paediatrics. The Social Worker will also engage in resource linkage and service coordination, consultation and collaboration with internal and external service providers in order to optimize the health and functioning of clients and their families.
    The part-time contract Social Worker works days, Monday through Friday, with occasional statutory holidays. The physical demands of the role may include but are not limited to prolonged sitting, standing, walking, bending, lifting, pushing and pulling. This position contributes to ensuring there is a supportive environment for patients, staff and visitors.

SKILLS AND QUALIFICATIONS:

  • Masters preparation and registration with the Ontario College of Social Workers and Social Services Workers Social Work (MSW, RSW) required.
  • A minimum 3 years work experience in a hospital setting is preferred.
  • An in-depth working knowledge of the psychological impact of chronic illness on the person and the family.
  • Working knowledge of the health system and relevant legislation (i.e. Mental Health Act of Ontario, Child & Family Services Act, Health Care Consent Act) is required.
  • Ability to provide crisis assessment and intervention, including suicide risk assessment, prevention and intervention is required.
  • Strong skills in community resource linkage, and collaborative and innovative care planning within a multidisciplinary team is required. Familiarity with related local community resources is an asset.
  • Proficiency working on a team of multidisciplinary healthcare professionals is required. This includes strong verbal and written communication, mediation and teamwork skills.
  • Proficiency in self-directed application is required. This includes demonstration of independent initiative and highly developed skills in organization, time management, and problem-solving.
  • Proficiency with electronic record-keeping and database applications, such as Meditech, is an asset.
  • Occasional evening and statutory holiday work is required.
  • Demonstrated ability to attend work on a regular basis is required.
