Social Worker (Senior Care Centre)

• Assess seniors’ care needs and formulate individualized care plans.
• Develop and implement programs for the elderly through regular data analysis, coordinate care with stakeholders, and document case notes.
• Facilitate networks, service referrals, participate in case discussions, and provide case management.
• Implement and evaluate new programs, review and initiate improvements.Guide volunteers, manage budgets and resources, and collaborate with other organizations to deliver programs effectively.

JOB REQUIREMENTS :

• Diploma/Bachelor’s Degree in Social Work.
• Preferably relevant experience in Social Service sector, working with elderly clients or any related field.
• Proficient in Microsoft Office (MS Word, Excel and PowerPoint).
• Able to work independently and in a multidisciplinary team to meet deadlines.
• Good interpersonal and communication (oral and written skills).
• Good knowledge about the community resources to engage stakeholders.
• Possess some knowledge of programme design, implementation and evaluation.Willing to work Mon to Fri 9am - 6pm.
