This is a Public Services Network (PSN) role and requires all successful applicants to undergo a Basic Disclosure check and a 3 year’s verification of employment history. YOUR ROLE

We are a short-term service providing support to service users after discharge from hospital, working closely with the reablement service as part of the discharge to assess model.

As a Social Worker in the social care assessment Service, you will:

• Hold a case load of clients, carry out Care Act Assessments
• Be responsible for Care Planning and case management of adults including being lead worker for safeguarding enquiries

You will be based primarily in Peasedown St John, Bath but will work across the whole of Bath and North East Somerset region.
