DESIGNATION : Social Worker

QUALIFICATIONS

  • 3 years experience as Social Worker
  • An accredited Registered Social Workers by the Social Work Accreditation and Advisory Board

Responsibilities:

  • Receive and assess referrals in conjunction with clinical team to determine suitability and ensure appropriate placement in Home/Centre. Make recommendations to ED or designate for approval of admission and funding.
  • Provide case management and psychosocial support services to residents, clients, and their families. Utilize appropriate frameworks for holistic assessment (e.g., BPSS model, Systems theory).
  • Identify and develop individualized care plans together with clinical care team to meet the psychosocial and emotional needs of residents and clients.
  • Address issues related to grief, adjustment, and coping with chronic illness or disability.
  • Implement and facilitate appropriate casework interventions, including ongoing monitoring and evaluation of outcomes.
  • Establish network with external partners and stakeholders for resourcing and ensure continuity of care of residents/clients across settings.
  • Work closely with multidisciplinary team to ensure that holistic needs of the resident/client are actively considered in their care planning.
  • Ensure hands-on approach to working with residents/clients through individual work, group work, counselling, or other appropriate modalities.
  • Maintain accurate and current documentation of casework for assigned caseload.
  • Work closely with residents, clients, and families to maintain or re-establish family relations when appropriate.
  • Provide support in end-of-life care to residents, clients, and their families. This may include bereavement support to family members of the deceased and practical matters such as referral to casket company for last rite matters, after office hours.
  • In compliance with government regulations for continued funding, conduct annual review of resident/client’s socio-economic status and make appropriate recommendations for financial assistance (e.g., deviation, Medifund) if necessary.
  • Participate in multidisciplinary meetings and work closely with Case Manager (Clinical) in discharge planning reviews.
  • Attend case conferences, meetings, network with external agencies as required.
  • Be a resource person for specific projects as needed.
  • Undertake any other related duties considered necessary, as assigned by SWM/ASWM or equivalent, in support of the department or Home.
