THKMC was incorporated on 13 October 2011 as a charity to provide multiple social and welfare services to the community at large, and achieved an Institute of Public Character (IPC) status on 18 November 2011. THKMC is the charitable arm of Thye Hua Kwan Moral Society, which lives by the mission To Serve Mankind to help anyone who needs help regardless of their race, religion, colour, language, creed and culture. THKMC today serves more than 70,000 beneficiaries through more than 70 programmes across five Services, which include Disability, Early Intervention for Children, Family, Seniors’ Community, and Therapy.

QUALIFICATIONS
- Degree in Social Work
Responsibilities:
- Managing potential client referral and conducting home visits to assess eligibility for intake into SCS services.
- Work with partner agencies and stakeholders, referring the clients and/or caregivers to those that can address their needs such as financial aid, healthcare, therapy etc.
- Providing information on details of services to new clients and their caregivers as well as family members.
- Engaging clients in a professional and respectful manner to understand needs and build rapport with clients and caregivers.
- Assist with the HH team on projects on basic to moderate palliative care needs clients.
- Compiling and providing timely submission of reports to the management and relevant external agencies for continuity of care
- Assist to conduct monthly financial waiver assessments and 6-monthly reviews to ensure the maintenance of well-being of clients.
- Attend to feedback and queries from clients and caregivers.
- Conduct planning, intervention, and case management.
- Conduct and participate in case conferences.
- Document and evaluate case outcomes through proper documentation.
- Other ad hoc duties if applicable
