Description:

If you are an experienced band 6 Dietitian looking to take the next step, with the passion and drive to make a positive difference in patients’ lives, join our team of Diabetes Specialists.
We are excited to invite applicants to join us for a Diabetes Specialist Dietitian role on a fixed term / secondment basis. This would be an ideal opportunity for either a band 7 specialist dietitian or an experienced band 6 dietitian looking to progress their career or gain valuable insight into the diabetes specialty.

Responsibilities:

Provide specialist, holistic support for patients with diabetes across the region with complex needs
Undertake nutritional assessment of patients and develop nutritional care plans which are both realistic and practical for the patient and carers, taking into account medical, social, cultural and psychological factors
Communicate clearly and sensitively with patients around nutrition, lifestyle changes and medication, particularly where there may be barriers to change
Actively participate in MDT meetings communicating expert nutrition and dietetic opinion with other health professionals to contribute to the most effective treatment and to improve patient safety and care
The successful applicant will share our commitment to providing patient focused care and will be an essential member of our supportive and friendly multi-disciplinary team.
The post holder will work closely with the multi-disciplinary Diabetes Team at Southmead Hospital to provide dietary advice and guidance on diabetes management to individuals with diabetes within our secondary-care diabetes service. Our service supports individuals with newly diagnosed type 1 diabetes, complex type 1 and type 2 diabetes (and other types of diabetes).
The successful applicant will provide patient-centred advice concerning carbohydrate counting, healthy eating and lifestyle, carbohydrate awareness, activity management, weight management, diabetes technology, and diabetes medication adjustment, as required. The role will involve providing education on the use of diabetes technology and interpretation of its data to support diabetes management. Support of other dietetic issues and / or liaison with other specialist services may be required.
Dietetic input will primarily be provided through one-to-one consultations via face-to-face, telephone and virtual platforms. There may also be some requirement to provide education through patient group education sessions.

To provide a highly specialist diabetes dietetic service, at an advanced practitioner level to Diabetes areas trust-wide.
To facilitate diabetes nutritional care as a key member of the diabetes multi-disciplinary team, by offering expert advice about diabetes-related nutrition to highly complex individuals with diabetes
To deliver specialist education, training and treatment on nutrition, lifestyle, medical and technological management to individuals with diabetes, their parents / carers, a range of healthcare professional and students.
To undertake audit and research related to Diabetes services.
To contribute to implementation of the Trust’s Diabetes strategie
