Specialist Exercise Physio (Heart Failure / Cardiac Rehab - REACH-HF)

To actively participate in the delivery and evaluation of national and local priorities for LVSD exercise provision. To work with support from the team as an autonomous practitioner, providing clinical expert advice and support to patients/carers and the wider multi-professional teams involved in the management of individuals with LVSD Act as a source of information and provide advice regarding patients care and management to medical, nursing and therapy colleagues within the healthcare setting. Responsible for policy development within own specialist area. Work with the Service Development Manager to ensure planning of the workforce and future cardiovascular exercise provision in a cost-effective way.
Work closely with fellow Heart failure Nurse Specialists within the team, Cardiologists, GPSIs in cardiology and Cardiovascular Nurse Leads to ensure local and national targets for heart failure are met
