Responsibilities:

ABOUT THE ROLE

Our new Clinical Care Program is aimed to provide greater access to mental health services for those people living with mental illness in the Central Coast region. Mental Health Services in Primary Care will improve service integration and achieve alignment with a person centred, stepped care approach. The aims will be achieved by engaging a range of mental health professionals to provide a suite of evidence based mental health services to people that would otherwise have little or no access to such services.
You will provide coordinated clinical care for people with severe and complex mental illness. The care will be delivered in line with a GP Mental Health treatment plan or equivalent, developed by a GP or Psychiatrist. You will supervise a Case Worker and Support Worker in the team and together, will deliver person centred holistic supports to clients on their mental health journey. You will collaborate with GP’s, psychiatrists, and other allied health professionals, as well as family members or support people to ensure that clients are able to move forward on their journey with the support they need.
This is a permanent role based on the Central Coast and covers participants who on the Central Coast. CatholicCare is open to flexible working arrangements that meet the needs of the program.

KEY RESPONSIBILITIES:

  • Good working knowledge of mental health, local networks, legislation and practices.
  • Demonstrate excellent skills in implementing strategies that promote emotional and developmental well-being in people who are experiencing ongoing, complex mental health disorders.
  • Building relationships and collaborate with external stakeholders to provide a holistic approach to support for clients.
  • Supervising two employees.
  • Writing detailed and informed notes based on the needs of clients.
  • Promoting a ‘risk-aware’ culture where staff proactively identify, report and address all forms of risk to clients, staff and CatholicCare.
  • Ensuring compliance with incident management, reporting and escalation requirements and that organisational obligations under mandatory reporting, child protection, safeguarding and WHS are met.
  • Oversight of brokerage requests and approvals of financial assistance.

TO BE SUCCESSFUL IN THIS ROLE, YOU WILL HAVE:

  • Tertiary qualifications in a relevant area e.g.: Mental Health Registered Nurse, Psychologist, Social Work qualification with mental health.
  • Evidence of independent practice history and skill in meeting the range of demands of complex caseloads.
  • Demonstrated ability to manage crisis situations and make sound decisions under pressure.
  • Knowledge of current Child Protection, Crimes (Domestic and Personal Violence) Act 2007 and other relevant legislation.
  • Experience working in a client centred, trauma informed and culturally specific way
  • Experience in the managing a team, with a strong ability to build an effective engaged team with a positive work culture.
  • Demonstrated networking and advocacy skills with both internal and external stakeholders.
  • Experience in best practice audits with the ability to manage changing practices.
