Description:

The post holder will provide expert knowledge and skills as part of the Community Paediatric Medical Service (CPMS), specifically the Children Looked After Team, delivering statutory services in for children placed in the Southampton area. To undertake review health assessments for Children Looked After, in environments appropriate to the child/young person for example, clinics, home visits, voluntary sector and local authority venues. To provide clinical support for Child Protection medical assessments during periods of planned annual leave. Professional/Clinical Looked After Children To provide high quality holistic assessments and monitoring of the health status of children & young people, in accordance with national and local standards.
To develop personalised, robust health care plans that will enable good long-term health outcomes, improve placement stability, and reduce inequalities in health outcomes for care leavers. To undertake health assessments and produce health summaries and health care plans that identify the health needs of children/young people and any action that needs to take place during the following year. The reports need to be understood by social workers, birth parents, foster carers, young person, and other health professionals. To assist care leavers to gain independence and the skills to access mainstream health services and provide a health care plan on discharge from the service.
To undertake one-to-one and group health promotion sessions on identified health needs with young people or their carer’s, on topics such as, sexual health, drugs and alcohol, fitness, and emotional health and other relevant topics as required and in partnership with experts who may support the service in delivering on some subject areas. Child Protection Liaison with the multi-agency network and the wider medical team To provide clinical support on a rota for Child Protection medical assessments during periods of planned annual leave. General To promote effective liaison with professionals from health and other agencies at primary, secondary and tertiary levels. To ensure effective and appropriate care is in place, and where necessary, to communicate complex and highly sensitive information to children, young people and their carers including those with learning difficulties or where English may not be their first language.
Ensure that contemporaneous, accurate and clear records are made at all times. To use specialist clinical skills in assessing and analysing need, referring, and developing care plans for children and young people in complex situations. To empower parents/carers/young people to access appropriate health provision for children and themselves, offering support and information when required. To act as an advocate for children and young people ensuring their views relating to their health & wellbeing are heard.
To provide advice regarding appropriate use of primary care and other health services. To support the teaching programme delivered to Medical Students by CPMS, and to participate in other teaching opportunities within the Trust as they arise. Management and Leadership Responsibility for ensuring that: Provide supervision and support, where required, to junior members of staff and other bands. Staff are supervised appropriately, and standards of work are maintained and Trust policies for the management of staff performance are adhered to.
Staff receive appropriate health and safety and other mandatory training as prescribed and that relevant records are maintained. Other records are maintained by the team ensuring compliance with the agreed record keeping systems of the Trust. Informed consent is obtained where appropriate and documented in accordance with Trust procedures. New and replacement equipment is not used until it has been commissioned in accordance with Trust policy.
Appropriate risk and other assessments are completed and documented in a timely manner in accordance with Trust policy. Defective equipment is reported and made safe without delay in accordance with Trust procedures Equipment is maintained in accordance with Trust policy. Infection prevention and control procedures are followed by all staff within their area of responsibility All staff follow Trust clinical governance requirements. Administrative Work positively with colleagues to maintain effective relationships, incorporating Trust values.
Contribute to annual report for Solent NHS Trust, highlighting the effectiveness of health care planning and progress toward performance indicators. To implement, participate in and analyse audit within the service to improve future service provision. To participate in ongoing research and development within the team as appropriate. Ensure mandatory training is accessed as detailed by the Trust policy.
Engage in annual appraisal process to support individual career and personal development. Education Working within the CPMS; the Named Nurse for Children Looked After, Clinical Nurse Specialist or equivalent professional will provide direct supervision and mentoring. Be aware of own professional development requirements and undertake learning and development opportunities. Attend education and training courses relevant to field of practice in order to further develop competencies.
To develop skills relevant to area including developmental assessment using recognised tool, history taking physical assessment and examination skills. Limits of Authority May not dismiss or suspend staff under the disciplinary procedures. May not take annual leave without prior agreement of the manager General Requirements Ensure they and where appropriate their staff: Adhere to trust policies and procedures at all times, Comply with trust standing orders, standing financial instructions, policies, procedures, and guidelines. Follow any policies and procedures in relation to infection, prevention, and control.
Are aware of their responsibilities in relation to safeguarding children, and vulnerable adults, and the specific responsibilities placed on individuals who care for such clients/patients. Take all reasonable steps to manage and promote a safe and healthy working environment which is free from discrimination. Comply with the trust policy on confidentiality, and the Data Protection Act 2018 as amended, relating to information held manually or on computerised systems. Respect the confidentiality and privacy of clients and staff at all times.
Maintain a constant awareness of health, welfare and safety issues affecting colleagues, patients, visitors, and themselves, reporting any accidents or fault in line with trust policy. Fully participate in health and safety training. Participate in personal training, development, appraisal, and attend all relevant training courses as required. Comply with the professional body code of conduct.
