Description:

Due to internal staff changes we are pleased to offer this exciting opportunity for an experienced Speech and Language Therapist to take up the role of Locality Coordinator for the South East Northumberland Community adult SLT team.
We are looking for an innovative, motivated and forward thinking individual to work part time as a Highly Specialist / Locality Co-ordinator SLT supporting service deliver to adults with acquired communication and swallowing problems in the South East Northumberland area.
This post forms part of the adult SLT team who pride themselves in providing a joined up high quality service for clients which involves team working at all levels of service delivery.
This post is based at Blyth Community Hospital, but may also involve work at other hospital sites within the Trust.
The post offers an exciting opportunity to provide highly specialist assessment and treatment to clients with a range of eating and drinking and / or communication difficulties in residential / care settings, in their own homes and/or via outpatient appointments.
In addition, the post-holder will be responsible for co-ordinating community SLT service delivery in the South East Northumberland area, including caseload prioritisation and monitoring, staff and student support and supervision and addressing day to day queries.
We manage three major locality hospitals in North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state of the art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. we also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate. high quality patient care is at the heart of everything we do and we strive to ensure every single patient and service user has an exceptional experience with us. we had one of the most extensive patient experience programmes of any trust in England.

Responsibilities:

independently assess, diagnose and treat own highly specialised caseload of clients and maintain associated records
participate in research in your specialist area
take a leadership role in the specialist clinical area offering advice to other therapists/staff on SLT issues
provide clinical co-ordination, support and supervision to the community locality SLT team, integrating goals of the team into the wider service area objectives
improve and maximise the communication and/or feeding skills of clients through liaison and joint working with staff, internally, externally and across agencies to enable creation and maintenance of local flexible service provision
The successful candidate must be able to meet the travel requirements of the post, which involve travel within the community and to other NHS sites across the Trus
