To provide an urgent 2-hour response to patients in the community to prevent hospital admission. To undertake assessments and coordinate and provide therapy intervention to facilitate and support a safe discharge within a 24-hour timeframe. To undertake specialist clinical therapy, including work requiring expertise or ability. To promote integrated, multidisciplinary working practices that supports seamless care.
