Description:

Attend team meetings, support the triage function of the team, support the duty workers with urgent clinical situations, undertake new patient reviews, including completing formulations and risk assessments for these patients,beavailableforclinicaldiscussionswithteammembersaboutpatientsontheteamcaseload,holdfollow up reviews of patients on the postholders case load Takeajoint leadinthemanagementofcomplexcases along with the consultants. Provideclinicalleadershipofteam,engagementwithMDTandRAMM Provideassessmentsofnewpatientsandformulatetreatment/careplans. Supportthetriagefunctionsoftheteamifrequested,withmedicalexpertise. Touseevidence-basedtreatmentandfollowlocalandnationalguidelines.
To work collaboratively and liaison with other teams within the Trust, other NHS organisations and third sectorproviders. TousetheMentalHealthAct,asappropriate,withinthecourseoftheirduties. Toworkinacollaborative,multi-disciplinary,multi-agencyandpartnershipway. Toprovidemedicaltreatmentwithinamodelofmulti-disciplinarycare.
Attendingweeklyclinicalteammeetingsandperformingmedicaldutiesthataredecisionsofthatmeeting. Compiling a patients history from a number of sources and preparing case summaries and discharge summaries where required. Preparing reports for Mental Health Review Tribunals and Managers Hearings and giving evidence in relation to patients on CTO. LiaisingwithotherclinicalteamswithinandoutsidetheTrust.
Liaisingwithgeneralpractitionersandmedicalspecialistswithregardsthephysicalhealthofthepatients. Assessmentofpatientsoutofareaonrareoccasions,thiswillbeinexceptionalsituationsonly. Attend ward rounds, undertake new patient reviews, including completing formulations and risk assessments for these patients,beavailableforclinicaldiscussionswithteammembersaboutpatients.
