Description:

The post holder (along with the other Tier 2 medical staff) will share in the provision of neonatal care at UHNM. Responsibility for day to day management of the NICU and all associated work will be shared by the Tier 2 team, under the supervision and direction of the appropriate service Consultant. Tier 2 cover is required on a 24 hour 7 day week basis. This would be minimum 1:8 rotational shift-based cover, or an agreed percentage, thereof.
The room 1 Tier 2 Doctor covers the intensive care room on the neonatal unit. They lead on delivery of care, supervised by the Neonatologist of the Week (NOW1) Consultant and would supervise Tier 1 medical care in the intensive care room. All Tier 2 medical staff cover this area on a rotational shift-based on minimum 1:8 basis. Consultants are resident until 9:30 pm 7 days a week.
Overnight, the tier 2 doctor will be the most senior member of neonatal team on site working along with a tier 1 doctor. Overnight, the neonatal consultant is non-resident : but provides advice over the telephone and if called in, can arrive on-site within 30 minutes. Out of hours the post holder will be the main tier 2 cardiac arrest and urgent bleep holder. There is significant consultant supervision and support available.
Shifts patterns are either 8:45 to 16:45, 8:45 to 21:15 or 20:45 to 09:15. During daytime hours (8:45 to 16:45 Monday to Friday), there are additional 1-4 Tier 2 staff who work under the supervision of either NOW1 or a second NOW2 neonatal consultant to support and cover the lower dependency areas, Weekend cover would include one tier 2 team member covering all areas (there is a plan to increase this to two), again with a resident single on call consultant until 9:30pm. One of this team would hold the cardiac arrest and urgent Tier 2 bleep in hours. A tier 2 doctor attends neonatal clinic held twice a week.
Outside the neonatal service shifts, the Speciality Doctor will have responsibilities for administration and development of the service, training of medical and nursing staff, clinical governance, risk management and clinical audit. These activities are included in SPA activity, for which dedicated time will be provided within the working pattern. The post-holder has the opportunity to develop their own area of interest such as renal clinics, leading on quality improvement projects etc. In addition, there are several learning/development opportunities: antenatal counselling clinic specialised developmental assessment clinics which is an individual consultant led follow-up clinics opportunities to scan or develop scanning skills Echocardiography is carried out as needed by 3 consultant neonatologists supported by 2 paediatricians with paediatric cardiology skills.
The post holder would have the opportunity to develop echocardiography skills and this service would be delivered as a team within a common governance framework. Cranial Ultrasound scans under supervision by one of the 9 consultant neonatologists. Attending a well established departmental teaching programme which includes weekly foetal medicine MDT, radiology MDT and several other trainee or consultant led teaching sessions attendance and participation in other specialised clinic such as foetal medicine clinics including interventional procedures, paediatric respiratory clinics etc. based on interest.
The Post holder will participate in a minimum8 person clinical rota with independent clinical activity. They will work under consultant supervision and there. At night, the Tier 2 role will include being the most senior clinician on site. There is over 12 hours per day on site consultant support and night time non-resident support.
