Description:

An exciting opportunity has arisen for a Registered Mental Health Nurse or Registered Learning Disability Nurse (Band 5) to join our existing team at Ty Skirrid Ward, Maindiff Court Hospital.
We are looking for an RNMH/RNLD who has commitment to working within the specialism of Forensic Rehabilitation and is innovative and enthusiastic, who can demonstrate an enthusiasm for the ongoing development of the service and who has a particular interest in psychological therapies and implementing groups.
You will need to possess good clinical knowledge and have an interest in working with individuals with complex mental health needs and be able to demonstrate an understanding of issues relating to this patient group. You will need to demonstrate a commitment to delivering high quality person centred care, contributing to the development and shaping of care delivery in accordance with national and local policy. You should have good communication skills and the ability to work effectively with patients, carers and other professionals. An understanding of risk management and the CTP process is expected.
You will be joining a team that is passionate and focused on achieving excellence in care delivery, for this challenging and rewarding patient group.
To maintain effective management of an agreed caseload acing as a care coordinator under the supervision of a senior case worker.
To work within the policies, procedures and guidelines set out by the ABHB and professional bodies. Operate within the guidelines set by the NSF, Clinical Governance and Risk Management.
To encourage and establish therapeutic relationships with individual service user’s and where appropriate their relative, carers and representatives.
under the supervision of the senior case worker to complete comprehensive risk assessments and risk management plans in accordance with Trust policy and guidance.
To provide clinically effective, therapeutically focused and evidence based care for service users individually or in groups.
To undertake specific clinical interventions, monitoring observations, aseptic techniques, undertaken tests and measurements e.g. blood sugar, urinalysis, and depot injections.
To maintain multi-disciplinary working by utilising other professionals to enhance care and provide information and feedback for multi-disciplinary review and referral meetings.
To complete clear, accurate, concise and up-to-date clinical records and documentation relating to interventions undertaken with service users.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Responsibilities:

The post holder is a member of the In Patient Mental Health Team and will be responsible for the care coordination of a caseload of service users who are experiencing severe mental health difficulties under the supervision of the senior case worker. The post holder will take responsibility within their caseload for ensuring the assessment of health and social needs, formulation, planning, delivery and evaluation of care, support and treatment by the team, for both individuals and groups of service users including risk assessment and management, CTP, and care reviews. They will work collaboratively across a wide ranging multi-disciplinary services and agencies to provide effective delivery of health and social care, supporting a programme of recovery and social inclusion within the area.
The post holder may also supervise junior staff who are engaged with service users within their caseload
