Staff Nurse Children's Community Team

at  Calderdale and Huddersfield NHS Foundation Trust

To be responsible for the assessment, planning, implementation and evaluation of evidence based programmes of care for the child, young person and their family within the community environment. Provide care at home or within a community setting without supervision to facilitate the prevention of admission or reduction in hospital stays. Liaise directly with the acute ward setting, the multidisciplinary team and other agencies to facilitate seamless high quality care to the child and family at home and in the wider community
