Staff Nurse (Homecare)

Responsibilities:
1. Deliver safe, individualized nursing care adhering to standards in documentation, treatment, and infection control.
2. Manage complex patient care, assess conditions, and implement personalized care plans.
3. Coordinate patient care, liaising between physicians, patients, and families.
4. Build rapport with patients and caregivers, assess home care needs, and allocate resources.
5. Administer medications, perform wound dressings, and maintain accurate records.
6. Participate in Multidisciplinary Team meetings and prioritize patient safety.
7. Uphold confidentiality and handle inquiries professionally.
8. Adhere to policies, regulatory standards, and commit to professional development.
9. Maintain workplace organization and cleanliness.
Requirements:
1. Diploma in Nursing from a local Polytechnic, registered with the Singapore Nursing Board.
2. Experience in hospital or hospice settings preferred.
3. Proficient in IT systems.
4. Passionate about delivering excellent customer service.
5. Meticulous, with strong interpersonal and communication skills.
6. Team player who can perform under pressure.
7. Cert in Oncology and Palliative Care would be an added advantage
Home Care Work Scheme: Mon – Fri (must be comfortable to work on weekend/PH if scheduled to be on-call)
Working Hours: 8am-5.30pm
