Description:

POSITION SUMMARY:

Provide individual and group counseling and case management services to individuals with Substance Use
Disorders (SUD) so they may achieve sustained remission from SUD, restore and increase healthy social
connections, obtain stable income and permanent housing, and reduce their risk of homelessness.

EXPERIENCE:

  • Must have at least 2 years’ experience in SUD Counseling or Social Work.

CERTIFICATION(S) & REQUIREMENTS:

  • CPR and First Aid certification and Safety training required within 30 days of hire.
  • CDCA (Chemical Dependency Counselor Assistant), CDCAII, LCDC II (Licensed Chemical
    Dependency Counselor II), SWA (Social Work Assistant)

TECHNICAL SKILLS AND KNOWLEDGE:

  • Must have successful experience facilitating SUD group counseling.
  • Must have extensive knowledge of Substance Use Disorders and other issues affecting the

homeless population.

  • Must have basic computer skills, using Microsoft Office applications.
  • Should have working knowledge of OHMAS, CARF and VA program compliance standards.
  • Bilingual (Spanish/English) preferred, but not required.
  • Demonstrate commitment to the mission, cause of the YMCA, and uphold its values and ethics.
  • Ability to work effectively work with people of different backgrounds, abilities, opinions and

perceptions

  • Willingness and ability to be a mandated reporter for any signs or suspicion of child abuse and

neglect. All employees are required to follow the child abuse and neglect reporting process:
completion of Suspected Child Abuse/Neglect reporting form, calling children protective services
(696-Kids) and notifying your immediate supervisor, branch executive, risk management and
human resources department (s).

  • High School Diploma, Associates Degree preferred

Responsibilities:

  • Provide individual counseling, outpatient group counseling, intensive outpatient group counseling,

and case management to homeless individuals with substance use disorders.

  • Provide individual counseling and case management to a caseload of program participants.
  • Facilitate Outpatient Group Counseling and Intensive Outpatient Group Counseling.
  • Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record.
  • Establish and maintain regular collaborative relationships with social service organizations and

programs that residents may utilize for employment, housing, continuing recovery and other

needs.

  • Participate in daily team meetings, quality assurance meetings, in-service trainings and

coordination meetings.

  • Coordinate with Clinical Director on all critical clinical issues.
  • Willingness and ability to model the key attributes; Welcoming, Genuine, Hopeful, Nurturing and

Determined.

  • Other duties as assigned.
