Responsibilities:
ABOUT THE ROLE:
We are looking for an experienced, team-oriented primary care physician with a passion for and expertise in caring for Medicare and Medicaid patients in the community. This role will work collaboratively with other care team members to deliver exceptional, thoughtful care to a population with complex clinical and social needs.
HERE’S WHAT YOU’LL DO:
- Lead a collaborative care team, supporting up to 6 advanced practitioners, nurses, social workers, and clinical health advocates through case conferences, and clinical mentorship
- Direct patient consultation for high risk patients
- Travel across the assigned territory to visit local clinical teams and manage/supervise advanced practitioners
- Partner with in-house clinical specialists around the diseases/conditions that most impact our population
- Partner with the local operations leaders to oversee the design and implementation of programs that promote exceptional, personalized care, and achieving great patient experiences while driving down cost and improving quality
