CS#1’s aged care team has a focus to support older and vulnerable people in our communities to experience a sense of wellbeing, choice and independence whilst living at home, to feel they are a valued member of the community and have a sense of confidence accessing quality services and supports unique to them.
QUALIFICATIONS AND EXPERIENCE:
- Registered Nurse / Allied Health Professional with current AHPRA registration
- Demonstrated experience or knowledge of clinical support and/or case management experience in home care packages or aged care to support complex clients
- Comprehensive knowledge and understanding of the Aged Care Quality Standards, the aged care reform agenda and funding
- Demonstrated experience managing daily operations of aged care service programs or similar, including financial management, to deliver strategic outcomes and measurable operational improvement
- Demonstrated experience in developing and managing teams to deliver high quality service, and creating a culture of continuous improvement and empowering teams and individuals to speak up when they see opportunity for improvement
- Excellent written and verbal communication skills, including experience writing reports, policy and procedures, delivering promotional material and mediating positive outcomes for clients and teams
Responsibilities:
CS#1 is seeking a suitability qualified Registered Nurse or Allied Health Professional and/or with graduate qualifications in case management/allied health, to manage and lead a team of Case Managers, to deliver case management across the home care package programs.
This position is recognised as a key senior leadership role within the aged care services team and is central to the provision of operational leadership and management to ensure high quality, safe and meaningful client centred care.
Reporting to the Executive Director of Aged Care Services, the Support at Home – Clinical Manager is responsible for overseeing the day-to-day operations of our home care ppackage program services, with a view to manage, monitor and review the program, ensuring compliance with Aged Care Quality Standards, care and clinical governance framework, and the Home Care Package program.
The successful applicant will work closely with the Team Leaders and Coordinators to champion high quality and safe practice supporting continuous improvement. The successful applicant will also lead, coach and mentor their team to deliver quality outcomes for clients and build a strong and positive culture in line with our Purpose and Mission – Connecting people to enrich lives and provide services that enhance quality of life.
The Support at Home – Clinical Manager will have the support of the CS#1 Executive and Leadership teams to guide and support the Aged Care Services executive program in line with the program’s strategic direction and yearly KPIs.
- Permanent full-time position (76 hours/fortnight, core hours are 9.00am to 5.06pm)
- Annual salary from $102,554 – $107,079, commensurate with qualifications and experience, (Social, Community, Home Care and Disability Service Award level 6) plus superannuation and tax benefits to maximise take home pay
- Free parking at Head Office, based in Narrabundah
- Provision of a laptop
