We have a wonderful opportunity for an enthusiastic and proactive Support Worker Support Worker with a full clean driving licence to join the support team of a young man with a brain injury living with his family in the Dewsbury area.
You will support and contribute towards the planning and delivery of individual care, supporting in all aspects of daily living as our client has physical, sensory & cognitive disabilities and is wheelchair dependent.
Successful applicants will receive full support, training and supervision from the established support team and Case Manager.
Responsibilities:
- Assist the client to exercise choice and control over their own lives as far as they are able in everyday matters but taking into consideration cognitive problems
- Liaise with and support the family as required
- Assist the client with personal care as needed
- Work with the treating team of therapists (SALT, Physiotherapist & Occupational Therapist) to complete therapy programmes, motivate & engage the client in recreational & educational activities
- Supporting the client to use local amenities such as shops and leisure facilities and attend community events
- Follow the service user plan as documented for the client
- Maintain accurate and confidential records
- Participate in moving and handling procedures as specified for the client
- Maintain professional boundaries with the client and their family members, adhering to direction from the case manager as appropriate
