Our new discharge to assess service in Dunfermline enables people, following a stay in hospital, to be supported and assessed in their own home to determine the type and frequency of any care and support they may need to live as independently as possible at home.

At the British Red Cross, we recognise the importance of taking a holistic approach to the assessment of a person’s needs following a stay in hospital. Our discharge to assess service carries out these assessments in the home. Our goal is to ensure people get the right care, in the right place at the right time. By providing support and facilitating access to resources, we empower people to regain their independence and confidence, all while maintaining their dignity.
This is a Regulated Service and you will play a vital role in providing practical and emotional assistance to individuals in their own homes assessing their needs and conducting assessments to determine ongoing care needs.

Responsibilities:

Driving Requirement: A full driving license and access to a vehicle is preferred but not mandatory
