Team Lead Physiotherapist, Urgent Community Response - Virtual Ward

at  East Sussex Healthcare NHS Trust

To provide case management and intensive support to patients who are acutely unwell and on the Virtual Ward (VW). To provide an urgent response to patients in crisis and where an unplanned admission can be avoided. To undertake highly specialist clinical therapy assessments and treatments appropriate to Band 7 grade including work requiring expertise or ability. To be jointly responsible with the Team Leads / Matron within UCR / Virtual Ward for the delivery of services across the East Sussex Healthcare NHS Trust community footprint.
To fully integrate within the wider network of Therapy Services, working in collaboration with other Team Leads to ensure there is consistency and assurance across the services within East Sussex To work with the Service Manager and ACP to continually review, develop and refine pathways of care, facilitating the delivery of a robust, innovative, and expert service. To support the Physiotherapy Professional Leads with the development of professional standards associated with Physiotherapy. To develop and work towards a delivering a service that operates seven days 8am-8pm in line with other UCR pathways
