Description:

We are looking for a highly motivated Telehealth Officer for the My SENSE Programme working alongside High Intensity Users Team. The Award Winning team supports the introduction and implementation of assistive technology to support patients who are attending the emergency department and have above average number of admissions into hospital. The successful candidate will have come from an experienced administration, therapy, social, care giving background and will be able to demonstrate a track record of working competently with key workforce systems. They must have a passion for quality patient care and be committed to supporting the team to achieve this. The ability to communicate with a wide range of professionals is a must.
Day to day the successful candidate will be expected to provide the My SENSE function of the team. This is an ideal opportunity for a dynamic forward thinking individual who wishes to develop with the team and themselves on this exciting new development which is currently expanding. Responsibilities:

KEY DUTIES AND RESPONSIBILITIES:

  • Support the completion of monitoring and referencing: Dashboard, Patient Daily Tracker, Intervention Log
  • Utilise the SOP created.
  • Recruit and consent patients
  • Demonstrate good understanding of principles of consent and ensure valid consent is obtained
  • Demonstrate good understanding of the Care Act 2014 / Mental Capacity Act / Deprivation of Liberties and applies principles to everyday practice seeking advice / guidance from the Case Manager / Team Lead or registered care professional as required.
  • Delivery and installation of equipment
  • Perform reviews and complete observations if needed
  • Ensure the privacy, dignity and safety of individuals is maintained at all times
  • Demonstrate the ability to recognise changing priorities / conditions seeking advice and guidance from the Line Manager or other care professionals as appropriate.
  • Support individuals, providing person-centred care and promoting health and independence through awareness raising and care navigation to other professionals as appropriate.

JOB PURPOSE:

  • This post will be for one of the main Telecare Officer responsible for monitoring and facilitating for

MySENSE as part of supporting the expanding roll out of the programme to appropriate patients

  • The aim is to offer frailty patients the opportunity to have assisted technology MySENSE to monitor changes in health and behaviour against an agreed patient baseline. This will help to identify early support and aid patient confidence and avoid unnecessary escalations to the acute Trust.
  • This is part of a programme to reach an additional cohort of patients
  • Providing MySENSE and other assistive equipment to support to patients at home, escalating findings to relevant professionals as needed.
  • As part of this role, it is essential to have training for usage of observation equipment, full training will be provided.
