Description:
We are looking for a motivated and caring individual to work within our highly skilled and dynamic service working with specialist nurses and physiotherapists to deliver a high quality service to patients.
As a Band 4 practitioner in our team, you will be responsible for supporting patients to complete the cardiac rehabilitation programme by leading exercise classes and supporting patients virtually across the City of Bradford.
Main duties of the job
To carry out exercise programmes with patients without direct supervision
To assess patients response to interventions and decide appropriate action.
The post holder will be responsible for a manageable caseload of a patients with a variety of cardiac conditions. They will also take an active role in promoting self management and third sector services once patients have finished their cardiac rehabilitation programme.
The role involves working Monday to Friday.
The postholder is required to work as part of a team, working with therapists, nurses, patients and carers and other healthcare staff.
The postholder will be responsible for delivering defined therapy input without direct supervision, but will have immediate access to a qualified therapist for advice and direction as required.
The post holder will work across In Patient wards and in the community setting as a lone worker.
The postholder will be responsible for managing and prioritizing their caseload within the cardiac rehabilitation team as delegated by qualified physiotherapists and nurses.
To manage and organise the flow of patients through phase 3 of the cardiac rehabilitation programme, evaluating and adapting interventions, informing lead physiotherapist of any changes or developments in patient’s progress in a timely manner.
To aid rehabilitation and improve functional ability, education, self management and confidence of patients.
To work as an active member of the cardiac rehabilitation team.
To contribute to team and service development.
To provide support to other team members and students.
