Description:

The SMASH Care Community has an exciting opportunity for a suitably experienced forward thinking Community Therapist wishing to progress into a Team Leader Role. This leadership role requires you to manage and co-ordinate the Community Therapy Service, be professionally accountable for the delivery of patient care by therapists within your defined team and be responsible for supervision and quality of their work.
To work with the Care Community Service Manager in the line management responsibility for both human and financial resources of the Community Therapy Team.
Part of the role will require you to act as an advanced clinical specialist, responsible for the assessment and treatment of complex community patients who have a variety of conditions.
You will be providing advanced level assessment and intervention for patients with complex needs. Working within the therapy teams and wider CCICP multi-disciplinary teams, you will provide advanced level clinical advice to support integrated working for this patient group which requires you to be an experienced Occupational Therapist or Physiotherapist.
To act as a clinical lead for patients with complex health needs, supporting the delivery of expert clinical care and providing clinical leadership to all team members within sphere of responsibility.
To lead, manage and co-ordinate the Community Therapy Service and be professionally accountable for the delivery of patient care, by therapists within the defined team and be responsible for supervision and quality of their work.
To be responsible for implementing and monitoring adherence to agreed policies, protocols and clinical standards, ensuring the effective and efficient delivery of high- quality service provision within the bounds of clinical governance.
To act as a clinical lead for patients with complex health needs, supporting the delivery of expert clinical care and providing clinical leadership to all team members within sphere of responsibility.
To act as an expert source of knowledge, utilising your past experience of being a OT/PT offering specialist clinical advice and support to other healthcare professional and agencies across organisational boundaries.
To work autonomously to manage a highly complex patient caseload, whilst working as part of the multi-disciplinary team and compliance with the Trust values.
To undertake highly specialist clinical assessment, taking into account all relevant clinical information and clinical reasoning to formulate individualised evidence based treatment plans in agreement with the patient.
To monitor progress and review treatment plans and interventions to facilitate the achievement of goals.
Mid Cheshire Hospitals NHS Foundation Trust provides a full range of hospital and community services for people across East Cheshire (population 399K) and West Cheshire and Chester (population 357K). To take a lead role in focussing team resources to meet the objectives outlined in the Service Specification and identified health needs within the community.
You will support the delivery of expert clinical care and provide clinical leadership to all team members within sphere of responsibility. You will also be expected to work alongside other clinical specialists within CCICP therapy teams, be responsible for service development initiatives, for positive leadership within the team and continuing to build and develop services.
You will also be expected to participate in the current urgent community response weekend rota on a pro rata basis
